My signature below signifies that:

1. All information that I have stated in this application, including all supporting documents that I have submitted, are true and correct. I have voluntarily provided this information and supporting documents to PLDT Inc. (“PLDT”)¹ to facilitate the processing and evaluation of my application for a PLDT Home Postpaid Fiber Subscription (the “Service”).
   
   I acknowledge that, in providing the Service, PLDT utilizes manual, partially automated, and fully automated processes to enhance operational efficiency. Human oversight is incorporated at key stages to ensure proper system functionality.
   
   ¹ PLDT Inc. refers to PLDT, its assignees, and successors-in-interest.
2. I acknowledge that PLDT will require the validation of my credit history in order to evaluate my application for the Service. I therefore authorize PLDT to share all of the following personal data with TransUnion Information Solutions, Inc. (“TU”), a private credit bureau operating in the Philippines, to compute for my credit score:
   • Complete Name
   • Address
   • ID number and ID Type (driver’s license, passport, company ID, etc.)
   • Date of Birth
   • Phone number and type
   • Gender
   • Civil status

   I acknowledge that:

   1. TU has the sole control of the algorithm it uses to generate my credit score; and
   2. My credit score shall be used by PLDT solely to evaluate my application for the Service.
   I understand that generating my credit score through TU does not guarantee that my application will be approved if I am found to have a low credit score:
   1. In case I get a low credit score and I am also a Postpaid Subscriber of Smart Communications Inc. (“SMART”), I authorized PLDT to confirm whether my account with SMART is in good credit standing (i.e., up to date on payments for at least 7 months).
   2. In case I get a low credit score and i) I am not a Postpaid Subscriber of SMART or ii) my SMART postpaid account is not in good credit standing, as a final resort, PLDT may obtain other relevant financial documents about my credit information, such as from my declared employer or business, and from the banks, credit card companies, and other financial institutions and service providers with whom I have existing accounts.
3. In order for TU to provide quality credit reports and scores to its members by enriching its database, I acknowledge that I have the option to allow PLDT to share the following personal data and account information to TU on a monthly basis, as part of PLDT’s contribution as a member of TU’s private Credit Bureau:

   Personal Data

   • Full name (first name and last name)
   • ID number, and ID Type (driver’s license, passport, company ID, etc.)
   • Address and address type (business or residential)
   • Date of birth
   • Phone number and type (business or residential)
   • Gender
   • Civil status

   Account Information

   • Account number
   • Account status (Active, Redirected, Temporary Disconnected, etc.)
   • Account activation date
   • Latest payment amount
   • Credit limit amount
   • Outstanding balance
   • Interest and fees (if applicable)
   • Number of days past due (if applicable)
   • Total amount overdue (if applicable)
4. I understand that all personal data shared by PLDT to TU is done with both parties acting as a personal information controller. Hence, all personal data disclosed by PLDT to TU will be governed by TU’s Terms and Conditions and Privacy Policy (and not that of PLDT) to the extent that the processing is solely under the control of TU.
   
   For more information, I may refer to www.transunion.ph or contact dataprivacyph@transunion.com in case I have questions or concerns regarding TU’s personal data processing purposes and activities and my rights as data subject in relation thereto.
5. I acknowledge that PLDT uses optical character recognition ("OCR”) technology to validate the proof of identification ("POID") I submit. OCR facilitates facial recognition and automatically transfers the details of my POID to PLDT’s customer application database. I understand that OCR technology helps prevent another person from impersonating me.
6. I acknowledge that PLDT may temporarily park my service application in cases when:
   1. My preferred service is currently unavailable. I understand that I may choose to wait up to a maximum of 30 days for my preferred service to become available. If, after the prescribed 30 days, my preferred service is still unavailable, my service application shall automatically be cancelled, unless I update my application and avail a different service type;
   2. PLDT requires additional supporting documents to be submitted. I understand that my service application shall not be processed until these supporting documents are submitted by me. If I am unable to submit these additional documents within 30 days, my service application shall automatically be cancelled; and
   3. I have unpaid balances due to PLDT. I understand that my service application shall not be processed until I have settled all unpaid balances. If I am unable to settle payment for these unpaid balances within 30 days, my service application shall automatically be cancelled.
   I acknowledge that PLDT may contact me for updates or reminders within the 30 days that my service application is temporarily parked. I also acknowledge that PLDT may use my parked service application for another three (3) months after, solely for PLDT to plan for future growth, increase its areas of serviceability, and improve its Services.
7. When I authorize PLDT to publish my Name, Address, and Telephone Number in the Directory Listing (White Pages) by indicating my preference for publishing in my Service application, I understand that no warranties are agreed upon the information published in the case that I will be reached and accessed by any person or number.
8. I agree to complete my personal profile on the welcome email that PLDT will send me once my service has been activated. I understand that I need to accomplish and submit it within thirty (30) days upon receipt of the email.
9. I acknowledge that PLDT will send my Statement of Account (“SOA”) for the Service five (5) calendar days after the Statement Date via electronic mail, through the email address I specified in the application. I agree to hold PLDT free and harmless from any liability for my failure to receive the SOA despite it being sent to the designated email address.
   
   I understand that I will no longer receive a printed copy of my SOA unless I have stated otherwise by indicating my preference for SOA delivery by request in my application.
10. I acknowledge that there are some instances where PLDT may be required to disclose my personal data as part of its regular business operations and for the provision of its products and services. I understand that my personal data may be disclosed to:
    1. PLDT’s service providers, contractors, and professional advisers who help PLDT provide its products and services. This includes partner companies, organizations, or agencies, and their sub-contractors who provide outsourced functions within or outside the Philippines to help PLDT facilitate its products and/or services, provide the service on PLDT’s behalf, or to assist PLDT in analyzing how its Service is used.
    2. PLDT’s subsidiaries and affiliates with whom I also have an account. If I am, become, or apply to be a customer of PLDT’s subsidiaries or affiliates, I understand that PLDT and the companies involved may choose to use and share my personal data for any of the following purposes:
       - To facilitate my account opening or other transactions with the concerned company;
       - To validate, consolidate, or update my account information;
       - For research purposes or to design related products or services for my benefit and use; and
       - To comply with a legal obligation to which PLDT or the concerned company is mandated to comply with.
    3. Other companies to whom I have given my consent to share my information. For instance, when I register for products and services provided by other companies, they may request information from PLDT to verify my identity; and
    4. Other companies, such as a Debt Collection Agencies, to whom PLDT may sell my delinquent account that has already been recognized for accounting purposes as bad debt.
    I acknowledge that I may find a complete list of these third parties on www.pldthome.com/privacy-policy-partners.
11. I acknowledge that PLDT processes and discloses my personal data to law enforcement and government agencies, in cases when PLDT is required to do so for purposes such as:
    1. To assist public authorities. PLDT produces statistical insights from my use of its network and facilities to assist public authorities in planning for healthcare, disaster management, and other similar initiatives for public benefit. Whenever possible, I understand that PLDT aggregates and anonymizes this information to ensure my identity is protected and not disclosed;
    2. To comply with legal requirements. PLDT processes or discloses personal data as necessary to comply with applicable legal and regulatory requirements. For example, PLDT provides information to the Credit Information Corporation, in support of the objectives of Republic Act No. 9510, otherwise known as the Credit Information System Act;
    3. To comply with mandates of public authorities and duly authorized private persons.. PLDT adheres to reporting obligations, including those related to contact tracing and other information requirements as part of government responses to pandemics and public health emergencies. PLDT also complies with law enforcement agencies in ongoing investigations, provided that legal processes are issued by courts or other authorities of competent jurisdiction.
12. I acknowledge that PLDT processes my personal data based on its legitimate interests to function effectively as a business, and that this is only done when such legitimate interests do not override my fundamental rights or freedoms. Such interests, include, among others:
    1. To manage my Postpaid Subscription. PLDT collects and processes personal data related to my use of the Service for billing purposes and collecting fees for the products and services I use. PLDT also processes my personal data to verify my identity during after-sales transactions I initiate through its hotline or stores. I understand that my personal contact details and are included in an internal directory listing to facilitate communication with me and provide me with assistance.
    2. To continuously improve its business and operations. PLDT analyzes my use of its network and facilities to monitor and manage the quality and security of the network, train its staff, and plan for future growth.
    3. To continuously improve its products and services. PLDT collects, uses, processes, and analyzes how I use its products and services to understand how to improve them and identify relevant products and offers. This analysis includes maintaining a record of the volume and frequency of my use of PLDT’s products and services. I understand that PLDT also analyzes other activities, such as my participation in market research initiatives, my visits and transactions in PLDT stores, and my use of any of PLDT’s websites and mobile apps.
    4. To send me information about PLDT’s products and offers, including those of its subsidiary and affiliate companies. PLDT analyzes my use of the Service to:
       - understand the market’s needs, wants and trends;
       - recommend suitable products and services and send me such information through SMS, e-mail, and voice calls. I acknowledge that PLDT’s advisories may include location-based offers that are exclusively available in the area where my PLDT Home Service is installed; and
       - determine the effectiveness of its marketing efforts and initiatives.
       If I prefer not to receive these messages from PLDT, I can manage my communication preferences at https://my.pldthome.com/.
    5. To manage the security of its business operations. PLDT processes my personal data to conduct IT security operations, manage its assets, ensure my fair use of its products and services, and support business continuity, disaster recovery, and audit purposes.
       
       In pursuit of these interests, I acknowledge that PLDT’s activities include monitoring its network with automated tools such as firewalls, capable of comparing suspicious data transmissions with known threats to deter cyber-attacks and detect and prevent fraudulent schemes, including phishing and identity theft, against PLDT and me. During such monitoring, PLDT may analyze traffic data and, if necessary, details relating to my identity, contact information, service usage, device information, and communication content. Should I commit any cyber-attack, fraud, or similar schemes, PLDT may block my access to the Service without prior notice.
13. I acknowledge that there are additional ways that PLDT may process my personal data, and that these activities are completely optional. PLDT will only perform these activities if I give my specific consent:
    1. Create a personal profile about me. PLDT processes my personal and sensitive personal information, analyzing it alongside my usage of its products and services (including data regarding the specific websites and applications I may visit), to gain a comprehensive understanding of and anticipate my personal needs and interests. Additionally, I understand that PLDT may collect personal data about me from third-party sources such as PLDT’s subsidiaries, affiliates, and trusted business partners, to whom I have provided consent to share my information.
       
       This enhanced profile is created to better understand my preferences, allowing PLDT to deliver more targeted and personalized product recommendations, special offers, and promotions directly to me.
    2. Send me information about PLDT Home products and offers through third-party messaging applications. With my consent, PLDT may send me product information, offers, and promos via Viber, Google, Facebook Messenger, and other third-party messaging technologies.
    3. Send me advertisements from third parties. PLDT processes my usage information, personal profile, and contact details to send me customized offers and promotions from its third-party business partners for the promotion of their own products and services.
       
       I understand that my personal data is not shared with these third parties. If I give my consent, I only agree to PLDT sending me direct marketing offers from its trusted partners.
    4. Share my personal data and profiles with third parties. PLDT may share my data, usage profiles (how I use PLDT’s network, products, and services, including my credit, payment, and purchase information) and personal profiles (my preferences and interests based on my usage information) for the independent business interests of its subsidiaries, affiliates, and trusted business partners. I acknowledge that I can find a list of companies to whom my data and profiles may be disclose to at www.pldthome.com/privacy-policy-partners.
    Should I consent to any of these additional activities, I acknowledge that I can also freely object to and withdraw my consent at any time by visiting https://my.pldthome.com/.
14. I acknowledge that PLDT will keep all information pertaining to my account, including all personal data about myself, for as long as I continue to use PLDT’s products and services, and for a maximum of ten (10) years after termination.
    
    In the event that my application is not approved by PLDT, I acknowledge that I have the option to ask for the return of any information and documents I have provided. Should I fail to exercise my option within the retention period, PLDT shall securely delete my personal data.
15. I acknowledge that I am afforded certain rights in relation to my personal data under the Data Privacy Act of 2012 (Republic Act No. 10173), and that I am entitled (in the circumstances and under the conditions, and subject to the exceptions, set out in applicable law) to:
    1. Request access to the personal data that PLDT processes about me.
    2. Request a rectification of my personal data.
    3. Request the erasure of my personal data.
    4. Request the restriction of the processing of my personal data.
    5. Request portability of my personal data.
    When the processing of my personal data is based on my consent, I acknowledge that I have the right to withdraw such consent at any time by contacting PLDT’s Data Privacy Officer, or by accessing https://my.pldthome.com/. I understand that this will not affect the lawfulness of the processing that was carried out before I withdrew my consent or PLDT’s right to continue parts of the processing based on legal bases other than my consent. If PLDT is unable to provide me with another legal basis justifying the processing of my personal data, PLDT will stop the processing and delete my personal data.
    
    If I believe that my data privacy rights have been violated, I may get in touch with PLDT’s Data Privacy Officer through the contact details provided below. In some instances, I understand that PLDT may request for supporting documents or proof before it effects any requested changes to my personal data.

Changes to this Subscriber Declaration

From time to time, I acknowledge that PLDT may update its Terms and Conditions and Privacy Statement to comply with changes in applicable laws, to comply with government and regulatory requirements, to adapt to new technologies and protocols, to align with industry best practices, and for business purposes.

You will always be provided notice if these changes are significant and, if we are required by law, we will ensure to obtain your updated consent.

This Subscriber Declaration is effective September 1, 2025.
