Document and ID requirements:

1. Valid proof of identification
2. Existing Pay TV SOA (Statement of Account) or proof of payment (original receipt or digital copy)
   1. Should contain Pay TV Provider/Company Name
   2. Registered Account Name must be the same as the applicant’s name and name on proof of identification
   3. Date of Payment and coverage of payment must still be within 2 months from date of application with Cignal
   4. Proof of Billing/ SOA (Statement of Account) or proof of payment must be readable, visible, legible, and without damage
   5. Should not be altered in any way
   6. Should not be torn, wet, or stained
   7. For handwritten ORs, writing should be legible and clear
   8. For printed receipts, ink should be visible and clear
   9. For online receipts, screenshot should be clear
